POSITION SUMMARY
A registered nurse who, under the general direction of the Nurse Director at Woodridge, is responsible for bringing people together, advocating for the residents and the facility, and smooth transitions of care between external sites and Woodridge. Through the building of long term relationships, the Admissions Nurse/Liaison will identify and assess the appropriateness of patients for potential admission to the acute, subacute, and long term nursing care setting. The role has four primary functions: 1) admission process management; 2) team building; 3) liaison to other interdisciplinary care team members; and 4) leadership.
BASIC KNOWLEDGE
Demonstrated understanding of unit operations and motivational techniques. Demonstrated ability to work independently in an interdisciplinary environment. Commitment to own career development. Demonstrated ability to function as a member of a team. Excellent communication and interpersonal/behavioral skills. Customer service orientation, to include skills in problem solving.
MINIMUM REQUIREMENTS
Current unencumbered license as a Registered Nurse in the State of Vermont is required. Please refer to department-specific Pay for Performance expectations. A Bachelor’s of Science in Nursing or other related field is required, or evidence of enrollment in BSN program within six months of hire. Master’s Degree in Nursing or related discipline preferred.
