Key Responsibilities:
- Provide direct patient care, including diagnosing and managing acute and chronic conditions.
- Educate patients on self-management strategies for chronic illnesses.
- Order, perform, and interpret diagnostic tests to guide treatment plans.
- Collaborate with senior care facility teams to ensure optimal outcomes.
- Maintain thorough and accurate patient records.
- Prescribe medications and recommend therapeutic interventions as needed.
Qualifications:
- Valid Nurse Practitioner license in Illinois (required).
- Minimum of 6 months of clinical experience (preference given to candidates with 1 years of experience).
- Experience with geriatrics, urgent care, or chronic disease management is a plus.
- Proficiency with EMR systems such as DocNow or Tebra and tools like Dragon Medical One is desirable.
- Reliable, insured transportation for travel between facilities (travel typically within a 20-40 mile radius).

Work Schedule:
