Position Summary
Supervises, coordinates and leads the shift activities of a defined team assuring adherence to appropriate service line agreements and escalation plans. Coordinates, prioritizes and optimizes patient throughput, transfer acceptance and consultation for UNMH Health System. Supervises staff covering various shifts on a twenty four hour, seven day per week basis. Participates in employee selection, on-boarding, discipline, termination and performance review process. Serves as an internal and external customer representative to foster relationships and maintain patient access to specialized care needs. May assume lead in absence of manager. Ensure adherence to Hospital and departmental policies and procedures. No patient care assignment.

Detailed Responsibilities
- SUPERVISION - Develop efficient organizational structure. Supervise employees and select, terminate, train, educate, correct deficiencies, perform appraisals, issue discipline, counsel, schedule work assignments; encourage staff teamwork and growth initiatives
- TRANSFERS - Facilitates acceptance and transfer of patients into UNMH Health System; coordinates transport needs (air and ambulance) at the request of the referring facility by use of service line agreements; communicates bed assignments in a timely manner to referring facilities
- SOFTWARE - Expert in use of Transfer Center software. Using provided software tools will collect clinical information; collaborate with Amion (on call providers) for access to specialized care
- CUSTOMER SERVICE - Maintains customer service elements at all times to include: update on bed status, follow up on issues and professional standards of communication; ensures all elements of Transfer Center information are accurately completed
- QUALITY - Participates in Quality assurance and education of information and call recording
- PLACEMENT - Oversees and participates in patient placement and bed assignment functions: Expert in use and working of TeleTracking XT software with understanding of interfaces; all patient assignments are made in the Teletracking XT system or Transfer Center Software, both of which are downstream from the ADT Cerner system
- RESOURCES - Utilizes community resources through partnerships for patient placement and transfers
- REVIEW - Reviews blocked beds with reason codes each shift and as needed; monitors inpatient bed status and upgrades EVS and transport tracking systems to expedite patient movement
- COMMUNICATION - Responsible for reading and responding to e-mail, collaboration of newsletters and posted information; integrates into daily work as appropriate
- ISSUE RESOLUTION - Escalates and documents issues quickly and appropriately within a specific timeframe
- CONSULT - Provides expert guidance, consultation, and leadership to an assigned team
- TRAINING - Leads, guides, and trains employees performing related work, may function as a preceptor
- COORDINATE - Coordinates services with patient care units; maintains communication with support services and maintains community relationships as appropriate
- QUALITY - Assists and may oversee quality assurance activities and compliance to operational standards
- EQUIPMENT - Ensures proper care in the use and maintenance of equipment and supplies; promotes continuous improvement of workplace safety, environmental practices and work environment
- DEVELOPMENT - Coordinates and participates in program development and implementation of operating policy and procedure
