What are the responsibilities and job description for the Home Healthcare Hospice RN position at UP Health System Home Care & Hospice?
Key Responsibilities:
- Makes the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
- Identifies the patient's/family's physical, psychosocial, and environmental needs, re-assessing as needed every 15 days.
- Initiates and coordinates the plan of care while maintaining accurate clinical notes regarding the patient's condition.
- Collaborates with the patient/family, attending physician, and other members of the IDG to provide daily patient and family care.
