About the Position
We are seeking a Clinical Care Coordinator to join our team at UP Health System – Marquette. As a Registered Nurse (RN), you will play a vital role in coordinating patient care and contributing to the well-being of our community.
Responsibilities
- Coordinate patient care, including patient transfer, discharge, referral, and spiritual/psychosocial support needs.
- Collaborate with other disciplines to ensure seamless patient care.
- Monitor patient condition and communicate effectively with the care team, physician, patient, and family.
Requirements
- A state of Michigan RN license and an associate degree from an accredited nursing school (bachelor's degree preferred).
- Basic Life Support Health Care Provider (BLS-HCP) certification.
- Advanced Cardiovascular Life Support (ACLS) and Pediatric Advanced Life Support (PALS) certifications within 1 year.
