Under the supervision of the HRSA PCHP (Primary Care HIV/AIDS Prevention) Program Manager. The (HSPS) HIV/STI’s prevention specialist, will provide HIV/AIDS, HCV, STI’s & PrEP comprehensive risk assessment, referrals, treatment and follow ups to LGBTQ/other patients reached by internal and external outreach activities. Conduct LGBTQ, UCHC staff and others individual and group prevention & education on HIV/AIDS, HCV, and STI’s & PrEP. Ensure high quality care to UCHC LGBTQ/other patients.
Special Projects:
- Support Covid testing and flu clinics at UCHC and in other community settings, such as schools, public housing, events by answering the phone lines, scheduling appointments, registering new persons being tested and checking in registered patients for testing
- Provide support to the Covid Testing and Clinic initiatives
- Outreach persons that ‘no show’ and educate patients and community members of the availability of special projects, such as Covid testing and flu clinics
- Perform remote laptop and label printer set ups as directed for successful remote clinics
- Perform Lobby Covid screening of patients, employees and persons walking into the main location
Duties & Responsibilities:
- Work as a team member to develop materials
- Conduct weekly outreach activities at various venues across Greater Boston Communities /Dorchester and Roxbury areas.
- Utilize social media for outreach
- Work in a flexible schedule (Some Nights and Weekends). As the program required.
- Maintain and secure client files.
- Be LGBTQ cultural sensitive and competence
- Conduct a comprehensive risk assessment to identify patient’s risks.
- Develop and monitor Risk Reduction plans for all patients.
- Implement /conduct individual and group sexual health behaviors educational sessions on HIV/AIDS, HCV, and STI’s & PrEP.
- Recruit and enroll patients into HIV/AIDS, HCV, STI’s & PrEP care/treatment, at UCHC or other local agencies.
- Conduct appropriate HIV/AIDS, HCV, STI’s & PrEP support referral. Internally and externally UCHC
- Provide referrals for services not offered at UCHC
- Build relationships and provide HIV/AIDS, HCV, STI’s & PrEP prevention and education services to patients and UCHC staff.
- Seek out and respond to opportunities that promote the knowledge, understanding and participation in the testing and services available at UCHC for patients with HIV/AIDS, HCV, STI’s & PrEP and substance abuse or patients at high risk for these.
- Establish and maintain relationships with community, local, and regional agencies and service organizations to coordinate access to testing and care for populations with HIV/AIDS, HCV, STI’s & PrEP and substance abuse.
- Establish and maintain relationships with community, local and regional agencies/organizations to coordinate access to LGBTQ services.
- Create and maintain effective working relationships with local and regional agencies/organization engaged in the work of HIV prevention.
- Conduct research activities related to HIV/AIDS, HCV, and STI’s & PrEP prevention.
- Participate in supervision, staff and agency meetings
- Collect, organize, enter and update program DATA
- Participate in meetings: Professional development, training, workshops and/or courses to increase individual capacity and industry awareness
- During Covid be willing to do tasks outside the scope of your job description
- Conduct other duties as assigned.
Qualifications
Experience & Qualifications:
- Bachelor Degree (HIV/AIDS, HCV, STI’s & PrEP)
- Or have at least 3 to 5 years working on HIV/AIDS, HCV, STI’s & PrEP prevention field
- Have been working or have familiar knowledge on HIV/AIDS, HCV, STI’s & PrEP prevention.
- Have been working or have familiar knowledge with LGTBQ community.
- Strong verbal/communication skills to recruit people.
- Strong knowledge computer skills
- Bilingual English-Spanish or English- Portuguese
