Position Title: Integrated Behavioral Health Clinician - Women's Health
Department: Behavioral Health
Supervisor: Manager of Behavioral Health
Status/Hours Per Week: Exempt / 40 hours
Primary Function:
The Integrated Behavioral Health Clinician is responsible for delivery of integrated behavioral health and case management services to the Women’s health OB/GYN Departments as well as Urgent Care and Primary Care. The primary function of the Clinician is to work with the patient and care team to improve the patients’ outcomes with their care plans. The brief interventions may include mental health and substances abuse screening, delivery of clinical, focused interventions, psycho-social assessments, linkages to the onsite social services for additional support or connecting to other Behavioral health department services as appropriate. The Clinician actively discusses care plans with Primary Care team members for continuing of care and provides short-term, time limited integrated services to primary care. The Clinician also maintains a small caseload of on-going patients for therapy services as needed and works closely with pediatrics and family services.
This position has a hybrid schedule flexible with both onsite shifts and remote shifts each week. This position provides trauma-informed brief intervention to individuals seen during their primary care visits.
Job Duties:
- Complete BH triage screenings for health center patients; provide needed crisis management, coordination for services and referrals.
- Coordinate and consult with women’s health treatment team on the recommendations and treatment needed.
- Provide counseling, either individual or family to a case load of patients.
- Participate in primary care behavioral health consultation during work hours, be available for walk in patients when scheduled for these open scheduled times.
- Coordinate any Women’s Health groups, psychoeducational materials/workshops to meet patient’s needs.
- Provide clinical case management, outreach, follow up and linkages to services as indicated for a patient’s care plan.
- Assist with coordinating patient care plans conducting ongoing collaboration with case managers including Community Health Advocates, Nurses and all other UCHC department staff and Providers.
- Participate in the OB/GYN case meetings and support the team in maintaining behavioral health portion of data records.
- Attend Behavioral Health team meetings, routine staff meetings, PC/OB team meetings for integrated care.
- Participate in Quality Assurance Performance Improvement projects, as needed for your program area.
- Participate in ongoing individual supervision.
- Maintain and complete in a timely manner all appropriate paperwork and documentation in compliance with the Behavioral Health Department policy and procedures for your program area, including use of the EHR and use of ICD 10 /DSM 5 coding as needed.
- Monitor patient follow up and clinical outcomes using a registry.
- Perform additional duties in the department as needed.
Qualifications
Minimum Basic Knowledge:
• Master's in Social Work with current LCSW licensure in the Commonwealth of Massachusetts, or LCSW within 6 months of employment start.
Experience & Qualifications:
• A clinical social worker/clinician with solid assessment skills.
• Interest in working with women’s health services and integrated care.
• Knowledge of trauma-informed care and integrated care is desired.
• Should be committed to community service, desire to work with a high-needs patient population.
• Good working knowledge of computers.
• Strong verbal skills, able to communicate well and work well with others both within the staff team and throughout other departments in an integrated care model.
• Bilingual in Cape Verdean Creole or Spanish is desired.
