Description
Engage participant and assist with personal care needs to participants across PACE Program care settings including restorative therapies, activities and meals service. This position works in the PACE Center.
Duties & Responsibilities:
The expectation is for PACE Center Aides to demonstrate competency performing the assigned tasks.
- Maintains collaborative, respectful, engaging and safe environment for enrollees, visitors and staff.
- Demonstrates the ability to engage with participants with diverse backgrounds, to anticipate their needs and encourage independence
- Assists participants with personal care. Responsibilities and duties may include but are not limited to ostomy care, skin care, incontinent care, simple dressings and wound care, vital signs, blood glucose tests and weights
- Carries out treatment plans under the direction and supervision of the clinical interdisciplinary clinical team to include behavioral, clinical and rehabilitation interventions such as functional transfers, positioning and mobility, nutrition plans, restorative therapies and recreation and engagement plans.
- Assists with implementation of the care plan determined by the clinical team under the direction and supervision of the PACE Center Manager
- Maintains confidentiality of enrollee records and reports and of all oral reports.
- Participates in working groups including Quality projects and maintenance of policy and procedure as assigned by the Center Manager
- Collaborates with the Center Manager in ways and means to establish better accountability of services to enrollees and referral sources
- Attends and participates in required trainings, staff meetings and enrollee care meetings.
- Able to respond effectively to changes in workload in order to fulfill episodic and emergent needs including escorting participant’s to appointments and assisting as needed with home-based midday service based on clinical needs
- Perform custodial and operational tasks including infection control practices, food and supply storage management and routine equipment maintenance.
- Able to complete basic administrative tasks as directed by supervisor including accessing the electronic health record to access and input relevant information.

Qualifications
Qualifications:
- High School graduate or equivalent
- Current certification as Home Health Aide, Nursing Assistant, or Medical Assistant from an approved program
- Enjoys working with older adults
- Flexibility, initiative, creativity, and good interpersonal skills
- Physical capability to lift and provide maximum assistance with transfers
- Physical capability to propel wheelchairs across all settings
- Ability to speak Spanish, Portuguese Creole, or French Creole a plus
- Is professional in appearance and manner in the clinical area; recognizes own limits and seeks help and guidance from the PACE Center Manager as appropriate;
- Responds in a positive manner to constructive criticism; serves as a role model for students, staff members, and new trainees.
- Maintains certification and pursues professional growth through continuing education process.
