This is a Partime position
Primary Function:
The Social Service Coordinator works under the supervision of the Director of Home Care, with specific clinical supervision provided by a LICSW as needed. As a member of the Home Care Team, the Social Service Coordinator provides direct service to patients and provides consultation to staff and families in accordance with Home Care policies and professional standards.
Duties & Responsibilities:
- Participates in the development of patient care plans and prepares progress notes as required by third-party payers and agency policy.
- Provides assessment, referral, and short-term intervention to patients and significant others.
- Consults with community based psychiatric/psychological consultants as needed.
- Provides crisis intervention services to patients and significant others for short-term emotional support/counseling.
- Regularly updates the interdisciplinary team on the status of social service intervention (for particular patients).
- Meets bi-weekly with team for updates on care plans.
- Supervises the Resources Specialist in the performance of duties specified in his/her job description.
- Collaborates with community representatives from home care corporations, Social Security, Medicaid, etc. as needed.
- Keeps staff updated on resources related to housing, community programs, alcohol counseling programs, etc.
- Participates in Community Task Forces as time allows.
- Performs other medical social worker duties as assigned by the Director of Home Care.
Qualifications
Master's degree in Social work
2 years of case management experience
