Position Title: Dietitian - Certified Diabetic Educator (CDCES)
Location: Upland Hills Health - Dodgeville Hospital Campus
Role & Department: Certified Diabetic Educator (CDCES) in the Clinical Nutrition Department
Hours & Shift: Full-time (1.0 FTE) day shift position, Monday through Friday 8 a.m. to 4:30 p.m.
Role Responsibilities:
- Will serve as the Diabetes Self Management Education (DSME) Program Coordinator for the facility
- Will oversee the planning, implementation and evaluation of the Diabetes Self-Management Program
- Is responsible for assessing the needs of each patient with diabetes and planning the care needed to meet those needs, including the spiritual, emotional and physiological needs
- Is responsible to see that the plan of care is carried out and coordinates the care of the patient by communicating with other members of the care team
- Will provide diabetic education and support to patients and families in both the inpatient and outpatient settings
- Will complete nutritional assessments and provide nutritional counseling to all UHH patients as needed
- Will participate and attend UHH meetings as needed
- Will design and participate in community nutrition programs as needed
- Will work with physicians, nursing staff and other ancillary staff professionals to provide high quality care for patients
- Must have a thorough knowledge of and utilize current clinical practices of medical nutrition therapy for diabetic patients
- Must have clinical knowledge to develop appropriate educational materials for patients served
- Will participate in menu planning for patients
- Must function as part of the health care team using interdisciplinary approaches in delivering care to patients/residents
- Must have good organizational & communication skills
- Must participate in organizational and departmental CQI activities
Qualifications:
- Candidates must have a bachelor or advanced degree in Dietetics from an accredited school
- Completion of ADA approved dietetics internship or equivalent education experience
- Be licensed as a Registered Dietitian in the State of Wisconsin
- Be a Registered Dietitian with the Academy of Nutrition and Dietetics
- Candidates must be licensed as a Certified Diabetes Educator (CDCES) or be CDCES eligible
- Knowledgeable of patient care standards according to the State of Wisconsin and in accordance with the Academy of Nutrition and Dietetics and American Association of Diabetes Educators (AADE)
- Must have academic preparation and/or experience in the care of persons with chronic disease
- Prior experience using the EPIC medical record system is preferred but not required
- Previous experience with the Microsoft Office Suite software programs is beneficial

Employment Bonus: This position is eligible for “up to a $5,000.00 Employment Bonus” which is prorated based on the FTE. Bonus payments will be made based on the FTE schedule below:
1.0 FTE $5,000
.9 FTE $4,500
.8 FTE $4,000
.7 FTE $3,500
.6 FTE $3,000
.5 FTE $2,500
The Employment Bonus will be paid out at set times during the first 3 years of employment in 6 separate equal installments. Those respective bonus payments will be made at the following periods: after completion of 6 months, 1 year, 18 months, 2 years, 30 months and a final payment at the 3 year anniversary. This Employment Bonus will be paid for Clinical Dietitian positions who are hired at a .5 FTE employment status or greater. Dietitian must continue to be employed at a .5 FTE status or greater at the time each of the bonus payment to be eligible to receive it.
