Position Title: Social Worker
Location: Upland Hills Health - Dodgeville Hospital Campus
Role & Department: Staff Social Worker in the Patient & Family Services Department
Hours & Shift: Part-time (.6 FTE or 48 hours per pay period) day shift position. Will work Wednesday, Thursday and Fridays for the 1st week of the payroll, and Tuesday, Wednesday and Thursdays for the 2nd week of the payroll period. Shift times are 8:30 a.m. to 5 p.m.
Role Responsibilities:
- The Social Worker will work with the Emergency Department, Outpatients, Acute Care, and Swing Bed patients & families
- The Social Worker collaborates with the interdisciplinary care team to provide patient education and to facilitate and coordinate patient services and post-hospital discharge plans
- This position is an integral member of the interdisciplinary team
- This position reports to the Patient and Family Services Director
Qualifications:
- Bachelor's Degree in Social Work (may consider a Bachelor Degree in Psychology)
- Hold current licensure as a Social Worker in the State of Wisconsin
- Prefer candidates with at least one full year of work as a Social Worker or in a similar type of clinical setting but this is not required
- Experience with the EPIC medical record system is beneficial but not required
- Previous experience with Microsoft Office Suite applications is beneficial
