Certificate Copy (Fundamentals of Coaching Education; Concussion in Sports; Sudden Cardiac Arrest and Heat Illness Prevention)
CPR / First Aid Certification (must be valid)
Driver's License Copy (Must be valid)

Letter(s) of Recommendation (Must Submit Two (2) Letters Dated within the last Twelve (12) months.)
Proof of HS Graduation (or equivalent)
