UPMC is currently hiring for a Full-TIme Assessment Counselor to join the outpatient Behavioral Health office located at UPMC Williamsport's Divine Providence Campus.

Purpose:
Assess and facilitate clients into Behavioral Health Services and other appropriate community services. Work closely with psychiatrist and other clinicians to provide quality care to all individuals requesting services
Responsibilities:
- Participate in meetings to facilitate continuity of care
- Assume responsibility for personal development and education
- Ensure complete and timely documentation of patient contacts.
- Act as intermediary with managed care companies as appropriate.
- Participate in Behavioral Health marketing efforts and represent Behavioral Health at community-based activities
- Commit to high quality, safe work; encourage others to have similar standards. Identify processes/systems that could lead to inefficient practices, unsafe conditions, errors or adverse events. Avoid shortcuts to ensure quality and safety measures are not circumvented. Encourage patient and family involvement in processes involving patient safety. Understand that patient safety is everyone's responsibility. Assume responsibility for personal education and performance related to patient safety; report unsafe conditions or events immediately; speak up in a non confrontational and educational manner when seeing a safety breech; develop or participate in a project or action plan to reduce risk or mitigate harm for patients, as appropriate
- Effectively interface with referral sources.
- Contact third party payors to obtain benefit information and to pre-certify treatment.
- Coordinate services with other units within the department, other hospital departments, physicians within the community, other service providers within the community and other mental health facilities.
- Know and follow all organizational/departmental policies/procedures.

- Assess and refer clients to appropriate level of care and collaborate with psychiatrists and other team members
- Coordinate accurate scheduling of clients with support staff
- Provide education to referral sources about Behavioral Health services and assessment referral coordinator role within UPMC Susquehanna.
Salary : $24 - $36
