We are hiring a full-time Behavioral Health Navigator to support UPMC Passavant-and St. Margaret!
The Behavioral Health Navigator I will be responsible for the assessment and navigation of patients and families in collaboration with attending psychiatrists and other staff within UPMC Passavant - McCandless and Cranberry as well as St. Margaret, either in person or virtually/telephone. They will develop initial treatment plans to facilitate disposition planning and assists with bed placement or disposition planning. They will have the ability to identify acuity level of patient and will facilitate the flow of information to promote effective communication regarding the patient care and patient operations and disposition.
While this position is based at UPMC Passavant - Cranberry, the incumbent will provide tele-services to UPMC Passavant - McCandless and St. Margaret. Occasional travel to UPMC Passavant - McCandless required. The incumbent will work three 12-hour shifts from 10:00 am to 10:30 pm as well as rotating weekends and holidays. Shift times may vary.
Responsibilities:
- Complete all required documentation and paperwork, including psychiatric evaluations, commitment paperwork, and insurance authorizations.
- Utilize DSMV for provisional diagnoses and review inpatient commitments for accuracy and compliance with mental health laws.
- Participate in interdisciplinary activities, maintain competency requirements, and use up-to-date technology for patient care assessment and referral planning.
- Handle a high volume of calls, efficiently meeting caller needs, and follow outpatient scheduling and referral documentation guidelines.
- Respond quickly to appointment or inpatient admission requests, review and summarize patient information for behavioral health unit admissions, and triage patients appropriately.
- Present cases verbally to physicians and others, demonstrating triage and assessment skills for medical necessity or appropriate outpatient clinic.
- Provide accurate, timely information to coworkers, physicians, and outside agencies in a professional and courteous manner.
- Access social systems to provide resource-specific information and facilitate patient/family referrals.
Salary : $22 - $33
