What are the responsibilities and job description for the Case Manager position at UPMC?
The Case Manager will work with individuals in Allegheny County who are diagnosed with an addiction to drugs and/or alcohol and are involved in the criminal justice system.
The shift for this position is 8:30 AM to 5:00 PM with potential opportunity to flex. Candidate will work primarily in an office setting but will be required to complete assessments a few times per week at the McKeesport Probation Office (approximately 2 blocks from office). The incumbent will join a small team dedicated to helping clients by maintaining a holistic approach.
Responsibilities:
· Maintain productivity according to program goals.
· Monitor client progress and act as a liaison between referral sources and treatment/community linkages.
· Attend staff development and training sessions.
· Assess treatment needs and characteristics of each individual.
· Develop and maintain community relations.
· Complete documentation, type correspondence, and compile statistics.
· Refer individuals to appropriate treatment programs and community resources.
· Assess case management and treatment needs.
· Collect and document urine specimens and breathalyzer results, and manage biohazard disposal.
· Develop individualized service plans for clients.
· Prepare monthly progress reports for referral sources, with input from treatment teams if applicable.
· Transport clients to appointments as needed.
Salary : $19 - $31
