Responsibilities:
- Patient Care: Provide focused nursing care and implement plans of care with holistic, patient-centered goals.
- Collaboration: Communicate with RNs and the healthcare team, seeking guidance when care exceeds scope, skill, or when patient conditions change.
- Adherence to Standards: Follow UPMC policies, LPN Practice Guidelines, and State Practice Act, including IV therapy administration.
- Documentation: Ensure comprehensive, clear documentation that promotes communication and continuity of care.
- Education & Support: Provide detailed teaching to patients and families for transitions in care; mentor new LPNs and support staff.
- Quality & Safety: Participate in initiatives to enhance patient safety, quality, and satisfaction, and identify opportunities for workplace improvement.
- Teamwork: Build strong relationships with patients, families, and colleagues to achieve optimal patient outcomes.
