A Nurse Navigator collaborates within an interdisciplinary team to deliver specialized care focused on wellness, prevention, illness, and rehabilitation. Acts as an advocate for patients and their families, serving as a connection to resources and services. Provides guidance and expertise as a clinical resource and consultant to the healthcare team while supporting staff development. Contributes to the assessment and enhancement of patient care, specialty services, and programs through quality improvement initiatives. This position offers a Hybrid and flexible work environment that blends clinical experience with school experience working within the community.
Responsibilities:
- Coordinates with the interdisciplinary team to guide patients and families through diagnostic and treatment options, providing emotional support, specialized care, and education.
- Acts as a care coordinator and patient advocate, delivering comprehensive nursing care across all phases of the healthcare process.
- Offers expert knowledge in specialty areas like women's health, breast health, or oncology etc, while addressing age-specific and cultural needs.
- Integrates research and technology into practice and contributes to quality improvement initiatives.
- Promotes health, disease prevention, and wellness education, serving as a clinical resource and supporting staff development.
- Plans and implements community health programs with a focus on specialty care.
Salary : $37 - $57
