This role involves working as a Residential Service Coordinator (SC) to provide on-site residential support as part of a multidisciplinary team. Clients live in Single Room Occupancy units on the 4th floor of the newly renovated Second Avenue Commons building.
The position involves coordinating mental health services for individuals with serious mental health diagnoses. Key responsibilities include assessment, service planning, coordination, referral, and re-evaluation to ensure community stability in relationships, housing, and employment. This work aligns with the Agency's vision, mission, values, and mental health Recovery Principles, emphasizing a recovery-oriented approach.
Responsibilities:
- Advocate for individuals to access necessary services.
- Participate in On-Call Service.
- Assess strengths and needs through interviews and reviews.

- Ensure monthly contact and provide comprehensive service coordination.
