What can you Expect as a Women’s Health Coordinator?

The Women’s Health Coordinator delivers care using interdisciplinary, collaborative, and standardization of system best practices in collaboration with the health care team applying expert clinical knowledge and skills to achieve quality patient outcomes; integrating operations and lines of business among the separate entities with leadership in women's health.

Responsibilities:

  • Participates in providing care to center patients. Applies best practice knowledge to improvement efforts. Practices solid verbal and written communication skills.
  • Develops relationships with service/specialty physicians and other disciplines to promote and proactively educate nursing personnel and patients on new therapies and procedures to enhance nursing care delivery and patient outcomes.
  • Promotes interdisciplinary partnership and consultation of hospital staff and health care team through participation in committees and related activities to ensure appropriate care and services for patients and families.
  • Utilizes appropriate computer systems to obtain data, enter and develop reporting tools
  • Demonstrates critical thinking in the analysis of clinical, social, safety, psychological and spiritual issues for the patient care.
  • Serves as a leader in clinical nursing practice, identifying patient and staff focused opportunities for improvement and leading the change process.
  • Supports programmatic documentation including forms, policies, procedures, correspondence and outcome measures.
  • Monitors dashboard indicators and quality in service line to meet organizational benchmark goals; develops action plans to address specific concerns and improve quality
  • Supports variety of special projects through project planning, coordination of efforts, and implementation.
  • Collaborates with medical staff to improve and monitor patient outcomes; sets clinical quality priorities, recommends and implements professional improvements and initiatives. Implements business unit-based patient safety initiatives.

Shifts/Hours: This position will work primarily daylight shifts and will be a hybrid schedule: working in the office and from home. Specifics of the hybrid schedule can be discussed at the interview.

Salary : $37 - $57
