What are the responsibilities and job description for the Case Manager - Adult PRP position at Upper Bay Counseling & Support Services, Inc.?
Upper Bay Counseling and Support Services (UBCSS) is seeking a Full Time Adult PRP Counselor who will manage a caseload of clients. The PRP Counselor provides supportive, individualized psychiatric rehabilitation services for adults with a serious and persistent mental illness. The PRP Counselor works within a multidisciplinary team to assess client needs develop professional and flexible ways to meet client needs, and implements interventions to assist clients in achieving their individualized rehabilitation goals. The PRP Counselor will provide on and off-site (community based) rehabilitative groups, support and activities and health home coordination related directly to client rehabilitation goals in accordance with COMAR regulations.
Candidates must be able to work as part of a team to coordinate care with our psychiatry staff, CMAs, psychotherapists, and administrative staff. Applicants should be organized, able to use an electronic medical health records, have strong computer skills, and be able to work independently.
Requirements: A valid driver’s license is required. A bachelor’s degree and experience working with adults with a serious and persistent mental illness is preferred.
Job Type: Full-time
Pay: From $15.86 per hour
Expected hours: 40 per week
