Upper Hudson Planned Parenthood is currently looking for a friendly, energetic, and reliable Patient Navigator to join our diverse team in our Albany Health Center.


This is a part-time (20 hours per week) grant-funded career opportunity that is eligible for our generous benefits package!

The Patient Navigator is a community health professional that supports the agency Patient Access program. This program focuses on the patient’s needs to access care for STI/HIV services on both an individual basis, and at a systems level.


This position is primarily responsible for supporting the Patient Navigation services for Upper Hudson Planned Parenthood and Central Western Planned Parenthood. This position will report to the Patient Navigator Manager for the direct STI/HIV patient navigation through the NYS Sexual Health Services grant. 


Some of the common day to day activities of the patient navigator may include scheduling appointments and/or referrals to other clinical care sites, coordinating health care activities of the patient, conducting financial intake and/or helping to secure funding for their STI or HIV testing, counseling and treatment if necessary and organizing logistical and practical support if necessary. The patient navigator will help patients identify needs and connect patients to appropriate resources and supports. The navigator may be involved and coordinate with outside entities as well. The patient may have needs to arrange legal counsel, transportation and travel logistics, dependent care or anything else necessary to help them obtain services. The patient navigator collaborates with diverse organizations and personnel to ensure the needs of the patient are met. 

The ultimate goal of the patient navigator is to relieve as much of the logistical and practical burdens from the patient as possible, such that the path to obtaining high-quality, patient-centered care and HIV services, is as clear and smooth as possible. As such, the role is also well positioned to identify, address, and improve the systems, structures and processes that create barriers to care at the level of the call center, health center, and ecosystem. This position usually involves daytime work, but sometimes night and weekend hours may be expected. Patient navigators may work with anyone seeking care that needs assistance. The patient navigator works closely with the health care providers, clinic staff, grant sources for HIV testing/counseling/treatment and other community organizations and volunteers.

? Deep commitment to patient access with understanding of and commitment to Reproductive Justice principles ? Ability to work on-site or remotely 

? Committed to values and practices of equity and reproductive justice 

? Ability to provide effective case-management to a varying caseload of patients 

? Ability to manage and advance both immediate, day-to-day details and needs as well as “bigger picture” strategies and goals 

? Experience and/or training in trauma-informed care, abortion access, case management, and/or program management a plus. 



An understanding and acceptance of Upper Hudson Planned Parenthood's mission, services, goals and programs. The ability to work efficiently and effectively, to prioritize, organize and perform detailed work accurately. Ability to work independently and proactively with regard to departmental needs. Excellent written, interpersonal and telephone skills. Knowledge of the VAN, Excel spreadsheets, Windows/Word, and familiarity with internet and web site use and maintenance preferred.
