- Service Coordinators are the backbone of our program. Vermont Comforts of Home (VCOH) offers adults a choice to remain in the community when they are no longer able to live on their own and also require a nursing home level of care.
- VCOH is a person-centered program.
- Service Coordinators meet at least monthly with the shared living provider and participant in their home to support and manage various aspects of the state-required expectations to ensure a healthy environment that meets the participant’s goals and needs.
- Mix of field work, work from home and main office in Essex Jct. at least two days per week.
- Field work located in the following counties: Orange, Caledonia, Washington
- Background checks are required
- Must have valid driver’s license and insured vehicle
- Generous benefits, holiday schedule and compensation package
- Thorough initial and ongoing training and support for Service Coordinators
- Mileage reimbursement provided for field work visits
- M-F 8am-4pm and occasional evening or weekend availability
- College degree in Human Services or related field preferred
