Job Description Job DescriptionPRIMARY RESPONSIBILITIES : The CM / Skill- building Professional works in partnership with the Youth’s core provider, specialty provider, residential provider, primary care physician, and other identified supports to develop an Youth-Individual Service Plan (IRP) which includes the medical, behavioral, wellness, social, educational, vocational, co-occurring, housing, financial, and other service needs of the eligible individualParticipate in initial assessment of client / family situations and ongoing monthly reports.The CM working in full partnership with the individual and other identified supports, leads the IRP implementation to coordinate the delivery of all services to maintain their recovery.o Locates needed services and makes referrals and arrangements for treatment and support services related to the Youth -Individual Service Plan;o Ensures the Youth gains access to needed services by assisting the Youth as he / she moves between and among services and supportso Actively assists the Youth / family to acquire needed resources including vocational, educational, transportation, entitlement benefits, housing, transportation, etcAdheres to all company policies concerning the preauthorization process.Participate in the service plan development within the initial thirty-day period via gathering information to incorporate into individuals’ Individualized Service PlansCM is expected to participate in planning, coordinating, and accessing services and resources when an enrolled consumer experiences an episode of psychiatric hospitalization, incarceration, and / or homelessness.Arrange for monthly case supervision review with Field Supervisor and attend Treatment Team meetings.Act as the liaison between family, referring agencies, and other community organizationsThe CM ensures the individual is receiving the appropriate quantity, quality, and effectiveness of services consistent with meeting the goals of the Individual Recovery Plan.Maintain case records and document services rendered.Attend mandatory staff meetings unless otherwise approved by managementParticipate in Discharge Planning meetings as needed.Maintain high degree of consumer confidentiality.Work with other staff assigned to cases, communicating effectively.Maintain a high degree of professionalism in the community, with clients, and with referring agencies seeking to build and maintain positive relationships.Participates in the development of a crisis plan between the individual, organization, core provider, and other providers where the organization is engaged with the individual to ensure that the plan is complete, current, adequate, and communicated to all appropriate parties;Assist in providing 24-hour crisis intervention as needed.Participates in Quality Improvement processes.

  • Monitoring / SurveillanceSupervisory Responsibilities : NoneLOCATIONAt least 50% of CM service units must be delivered face-to-face with the identified individual receiving the service and the majority of all face-to-face service units must be delivered in non-clinic settings over the authorization period .WORK SCHEDULE
  • The work schedule is flexible in order to meet the needs of the person receiving services. Services can be provided M-F, weekends, and holidays.
  • The CM must bill at least 30 hours weekly to meet full-time status.
  • This position does require the employee to be available for crisis response 24 hours per day, 7 days per week and 365 days per year.Job Requirements : Maintain good standing with state licensure and professional liability.
  • Must have strong therapeutic clinical writing skills and the ability to communicate effectively verbally and in writing.
  • Must have access to a secured computer, in which you are able to download ShareNote, EHR.
  • Excellent interpersonal skills and team player concepts.
  • Knowledge of strength based family and solution focused treatment strategies for working with clients at risk.
  • Maintain a caseload of clients with personal social behavior problems.
  • Must be self managed and have honest and reliable work ethics.
  • Must have valid Georgia Driver license and automobile insurance policy.
  • Must pass Criminal Background Check and Drug Screening.Note : Clinically Supervised by LMHPWithin the first sixty days all staff having direct contact with consumers shall receive the following training including, but not limited to : i. Person centered values, principles and approaches;ii. Holistic care of the individual;iii. Medical, physical, behavioral and social needs and characteristics of the persons served;iv.   • i. Nationally benchmarked techniques for safe utilization of emergency interventions of last resort (if such techniques are permitted in the purview of the organization);vii. Ethics, cultural preferences and awareness;vii. Fire safety (
  • ix. Emergency and disaster plans and procedures (
  • x Techniques of standard precautions, including : 1. Preventative measures to minimize risk of HIV;2. Current information as published by the Centers for Disease Control (CDC); and3. Approaches to individual education.xi CPR / AED through the American Heart Association or the American Red Cross.1. All medically licensed staff (nurses, physicians, psychiatrists, dentists, and CNAs) are required to have the Professional Rescuers level of training (Basic Life Support for Healthcare Providers and AED or CPR / AED for the Professional Rescuer).2. All other staff should have the Lay Rescuers level of training (Heartsaver CPR and AED or CPR / AED).xii First aid and safety;xiii. Basic cardiac life support (BCLS) includes both written and hands on competency training required.xiv. Specific individual medications and their side effects (
