Schedule: 4-day work week, 10-hour shifts, Monday- Friday

Position Summary:

As a Nurse Practitioner with Upstate Family Health Center, you'll deliver exceptional care to our patients, manage both acute and chronic conditions, and contribute to the overall health and wellness of our community. You’ll work alongside a supportive team of medical professionals, including dedicated support staff and a scribe to assist with documentation, ensuring that you can focus on what you do best – providing top-notch patient care.

Key Responsibilities:

• Patient Care: Conduct thorough assessments, develop treatment plans, and manage patient care, working closely with supervising physicians and other healthcare providers.
• Health Education: Provide counseling and education to patients and families, promoting wellness and guiding them through their health journeys.
• Documentation: Maintain accurate and timely clinical records in compliance with established protocols.
• Continuous Learning: Stay current with medical advancements and best practices to continually improve patient care.

Qualifications:

• Licensure: Family Nurse Practitioner License (FNP) in New York (training provided for qualified candidates).
• Experience: 2 years in a similar role, with strong skills in leadership, communication, and patient care.
• Skills: Proficiency in PCMH, clinical procedures, and effective interpersonal communication.
• Attributes: Empathetic, adaptable, and committed to high-quality patient care.
