What are the responsibilities and job description for the PEDIATRIC AMBULATORY CLINICAL CARE MANAGER position at Upstate University Hospital?
Job Summary :
The care management position is responsible for assisting patients and families to navigate their healthcare. Their primary focus is high risk patient populations within primary care pediatrics. Each role will encompass patient care as well as being the lead coordinator for all services related to the assigned patients; working closely with multiple agencies, specialty services and providers. This role will be responsible for the patient's individual needs as they relate to age, physical, psychosocial, cultural and education.
Minimum Qualifications :
R.N. NYS license or eligible, Bachelor's degree in Nursing or healthcare related field, plus minimum of three (3) years RN nursing experience required.CPR-BLS certification is required by the time of appointment.
Preferred Qualifications :
Master's degree in nursing or healthcare-related field. Pediatric and ambulatory experience preferred.
Work Days : Monday-Friday, Days
