What are the responsibilities and job description for the SCSD Intake & Discharge Specialist position at Upstate University Hospital?
Job Summary :
The Intake & Discharge Specialist is part of the school-based mental healthcare program in the Syracuse City School District. The primary role is to coordinate all intakes and discharges for SCSD students and families. The ideal candidate will work closely with the school-based team to field new patient requests, gather information from prospective and established students and families and process intake materials to ensure our program can meet the student's needs. The Intake & Discharge Specialist will also maintain a waitlist and work with community agencies as needed. This position will support the school-based mental healthcare program team including various quality improvement initiative and attend meetings as a vital part of the team.
Minimum Qualifications :
Bachelor's degree in Psychology, Education, Social Work or related field required.
Preferred Qualifications :
Prior experience working with the outpatient or school based mental health services.
Work Days : Monday-Friday days
