Job Description
The Care Specialist at Upward Health plays a crucial role in delivering comprehensive care to high-complexity patients. This position requires strong critical thinking skills, self-motivation, and the ability to work independently in an unstructured environment.

Key responsibilities include conducting direct outreach to patients via phone, in-person, mailings, and other strategies; enrolling patients into Upward Health's program; and collecting key data about patients during the enrollment process. The Care Specialist will also meet patients in their home or community to conduct needs assessments, facilitate in-home provider appointments, and coordinate care between patients and the care team.
This role requires a strong organization skillset, proficiency in time management, and the ability to prioritize tasks effectively. Excellent communication and motivational interviewing skills are necessary for engaging with difficult patients and explaining technical medical information.
Requirements
- At least 2 years of relevant work experience as a Community Health Worker, Peer Support Specialist, Medical Assistant, or similar role
- High school graduate or GED required
- A valid driver's license and auto liability insurance
- Reliable transportation and the ability to travel within your assigned territory or other as needed to support patient needs plan and training requirements
