About the Role
The Community Health Support Professional position at Upward Health is an exciting opportunity for a dedicated individual to make a positive impact in the lives of others. As a key member of our multidisciplinary medical group, you will play a vital role in delivering 24/7 whole-person care to patients in their homes and community.
Your primary responsibilities will include conducting direct outreach to patients via phone, in-person, mailings, and other strategies, enrolling patients into Upward Health's program, and collecting key data about patients during the enrollment process.
You will also conduct needs assessments to help patients set health goals, coordinate care between patients and interdisciplinary care teams as needed, and support patients in meeting their healthcare goals related to Quality measures.
To succeed in this role, you will need strong critical thinking skills, interpersonal savvy, and the ability to interact with and influence people to establish trust and build strong relationships.
