Job Summary
We are seeking a highly motivated and compassionate Care Specialist to join our team! As a member of our organization, you will play a crucial role in delivering high-quality care to our patients. Your responsibilities will include conducting direct outreach to patients, enrolling them into our program, and collecting key data about their health needs.
Key Responsibilities:
- Develop and implement personalized care plans for patients
- Collaborate with the interdisciplinary team to achieve market and/or organizational goals
- Document all patient interactions and updates in the EMR
- Provide education, resources, and referrals as needed to support patients in meeting their healthcare goals
Requirements
- Strong interpersonal skills and ability to interact with and influence people to establish trust and build strong relationships
- Self-starter with the ability to work independently in an unstructured environment
- Ability to complete unscheduled home visits, cold calls, and other outreach efforts as needed
