Community Health Liaison

We are seeking a highly motivated and organized Healthcare Outreach Coordinator to join our team at Upward Health. In this role, you will work closely with our clinical team to develop and implement outreach strategies that meet the unique needs of our patients.

Your primary responsibility will be to conduct direct outreach to patients, which may involve phone calls, in-person meetings, mailings, or other strategies. You will speak with patients about the benefits of our services, enroll them into our program, and collect key data during the enrollment process.

To succeed in this role, you must have excellent communication skills, be persuasive, flexible, and dedicated. You should also be able to establish trust with patients, communicate clearly, and respond to questions and objections.

In addition to your outreach responsibilities, you will participate in weekly team meetings to discuss ongoing education and improvement opportunities. You will also be responsible for maintaining accurate and timely documentation in our company's software system.

Key Responsibilities:

• Develop and implement outreach strategies to engage patients
• Conduct direct outreach to patients via phone, in-person meetings, mailings, and other strategies
• Speak with patients about the benefits of Upward Health's services
• Enroll patients into Upward Health's program and collect key data about patients during the enrollment process
• Manage a personal caseload of patients and ensure that each assigned patient is contacted using the most effective strategy
• Interact with enrolled patients to ensure ongoing engagement or to deliver additional community-based touchpoints
• Participate in weekly team meetings focused on ongoing education and improvement
• Accurate and timely documentation in our company's software system
