- At least 2 years of experience in chronic care management, community health work, or a related field
- Strong critical thinking skills, excellent communication abilities, and the ability to work independently
- Familiarity with concepts like Motivational interviewing, trauma-informed care, and care coordination
- Ability to manage and maintain a personal schedule, prioritize tasks effectively, and complete unscheduled home visits
- Valid driver's license and reliable transportation
- Ability to work within an interdisciplinary team to support market and/or organizational goals
