The Community Network Coordinator at Upward Health is a critical position that plays a key role in bridging the gap between healthcare and community-based services. As a member of our interdisciplinary medical group, this professional identifies and connects patients with community-based organizations (CBOs) that offer essential education, job training, and other forms of support.
Main Responsibilities:
- Foster relationships with CBOs, creating a network that caters to the diverse needs of Upward Health patients.
- Act as the primary point of contact for community partners functioning as CBOs.
- Maintain ongoing collaboration with clinical and community partners to ensure seamless patient care coordination when CBOs are involved in patient care plans.
- Assist clinicians and non-clinicians in monitoring patient health and identifying suitable CBO resources.
- Represent the community in which Upward Health's patients reside.
- Provide guidance to healthcare professionals and other stakeholders about CBOs available within the community, including their services, operating hours, and key contacts.
- Contribute to achieving team objectives by completing additional tasks as required.
Necessary Qualifications:

- Previous experience coordinating with cross-functional teams and external partners to meet specific patient or community needs.
- Background in social services.
