As a Health Advocate at Upward Health, you will have the opportunity to make a meaningful impact on the lives of patients and their families. Our team of advocates works tirelessly to ensure that patients receive the care and support they need to manage their chronic conditions and improve their overall health and well-being.
Key responsibilities include conducting outreach and engagement activities to educate patients about our services and promote enrollment in our program; working collaboratively with patients, families, and healthcare providers to develop and implement effective care plans; and providing ongoing support and education to patients and families to promote self-management and shared decision-making.
The ideal candidate for this position possesses strong interpersonal and communication skills, the ability to work independently in an unstructured environment, and a passion for helping others. A high school diploma or equivalent is required, as well as a valid driver's license and reliable transportation. Basic computer skills and the ability to type accurately are also necessary.
