The Role:
We are seeking a skilled Patient Engagement Specialist to join our team. As an Outreach Specialist, you will be the first point of contact for our patients and play a critical role in making a positive impression and effectively communicating our service offerings.
Your Key Responsibilities:
- Conduct direct outreach to patients via phone, in-person meetings, mailings, and other strategies
- Communicate with patients about the role Upward Health can play in improving their health
- Enroll patients into our program and collect key data during the enrollment process
- Manage a personal caseload of patients and ensure each assigned patient is contacted using the most effective strategy
- Interact with enrolled patients as needed to ensure ongoing engagement or deliver additional community-based touchpoints
- Participate in weekly team meetings focused on education and improvement
- Accurately document interactions in our company's software system
