About the Role
As a Care Specialist with Upward Health, you will play a vital role in delivering exceptional care to patients in their homes and communities. Your primary responsibility will be to conduct outreach and enrollment activities, ensuring that patients receive timely access to appropriate care.
Your day-to-day tasks will include coordinating care between patients and interdisciplinary teams, supporting patients in meeting their healthcare goals, and analyzing patient data to determine needs or treatment goals.
You will also be responsible for explaining technical medical information to educate patients, cultivating and supporting primary care providers with timely communication, and attending regular daily huddles, team meetings, and participating in clinical rounds.
We are seeking a self-starter with strong critical thinking skills, excellent communication and motivational interviewing skills, and proficiency in multiple electronic medical record (EMR) systems.
