As a Care Coordinator Specialist, you will:
- Conduct outreach and engagement activities with patients to promote timely access to care, increase preventative care utilization, and reduce hospital readmissions.
- Collaborate with interdisciplinary teams to coordinate care plans, facilitate communication, and ensure data integrity.
- Serve as a patient advocate by empowering individuals to take an active role in their health and wellness journey.
Your Qualifications
- A minimum of 2 years of experience in a related field, such as community health work or medical assisting.
- High school diploma or equivalent required; associate's or bachelor's degree preferred.
- Valid driver's license and reliable transportation.
