Key Responsibilities:
- Develop and maintain a network of CBOs serving Upward Health patients and their unique needs.
- Serve as primary contact for community partners acting as CBOs.
- Maintain a unified network of clinical and community partners sharing information for patient care coordination when CBOs are part of patient needs plans.
- Work with clinical and non-clinical staff to monitor patient health and connect them to relevant CBO resources.
- Represent the community where our patients reside.
- Advise providers and non-providers on CBOs throughout the community, including programs offered, hours of service, and points of contact.
- Perform other duties supporting goals and objectives.

- Strong organizational and presentation skills.
- Ability to coordinate multiple schedules and adapt to changing demands from various stakeholders.
Requirements
You must live within the patient service area and have:
- A valid driver's license and access to a vehicle with current and active insurance.
- Deep experience working in or with Community-Based Organizations.
- Able to work flexible hours, including occasional night/weekend work.
- Bachelor's degree preferred.
- Ability to travel within the patient service area.
- Strong verbal and written communication skills, with a talent for building trusting relationships.
