Your responsibilities will include conducting thorough assessments of patients' physical, emotional, and social needs; developing and implementing effective care plans; coordinating with healthcare providers to ensure timely access to appropriate care; and providing education and support to patients and families to promote self-management and shared decision-making.
To be successful in this role, you must possess strong critical thinking skills, excellent communication and interpersonal abilities, and the ability to work independently in an unstructured environment. A valid driver's license and reliable transportation are also required, as well as basic computer skills and the ability to type accurately.
