About the Role
We are seeking a motivated and empathetic Patient Outreach Coordinator to join our team. As a key member of our outreach team, you will be responsible for conducting direct outreach to patients via phone, in-person meetings, mailings, and other strategies. Your primary goal will be to educate patients about the benefits of Upward Health's program and encourage them to enroll in our services.
Main Responsibilities
* Conduct direct outreach to patients via phone, in-person meetings, mailings, and other strategies
* Educate patients about Upward Health's program and its benefits
* Enroll patients into our program and collect key data during the enrollment process
* Manage a personal caseload of patients and ensure timely follow-up and communication
* Participate in weekly team meetings focused on ongoing education and improvement
Requirements
* Excellent verbal communication skills and ability to build rapport with patients
* Strong persuasive abilities and ability to clearly communicate the value of our services
* Flexibility and ability to work independently and as part of a team
* Basic computer skills and ability to learn new software systems
* Reliable transportation and ability to travel to patient locations
