About the Job
As a Care Specialist at Upward Health, you will play a critical role in delivering exceptional patient care and support. You will work closely with an interdisciplinary team to achieve market and/or organizational goals.
Responsibilities
- Conduct outreach to patients via phone and in-person to inform them about Upward Health's services and facilitate enrollment.
- Meet with patients in their homes or community settings to conduct needs assessments, set health goals, and coordinate care with the care team.
- Facilitate in-home provider appointments and ensure that all assigned patients have initial provider visits and follow-up appointments scheduled.
- Support patients in meeting their healthcare goals related to quality measures.
- Work collaboratively with an interdisciplinary team to achieve market and/or organizational goals.
Requirements
- A minimum of 2 years of relevant work experience as a Community Health Worker, Peer Support Specialist, Medical Assistant, or similar role.
- High school graduate or GED required.
- A valid driver's license and auto liability insurance.
- Reliable transportation and ability to travel within assigned territory or other areas as needed.
- Experience in Chronic Care Management model or working with chronically ill/elderly patients.
- Long-time resident of the community with good knowledge of local resources.
- Ability to complete Upward Health's initial training program and ongoing educational requirements as assigned.
