- Identify and engage with CBOs that can provide valuable services to Upward Health patients.
- Build and maintain a network of CBOs, ensuring they cater to the diverse needs of our patients.
- Coordinate with clinical and community partners to facilitate seamless patient care coordination when CBOs are part of the treatment plan.
- Support healthcare professionals and non-clinicians in identifying suitable CBO resources for patients.
- Represent the community where Upward Health's patients reside.
- Provide expert guidance to healthcare professionals and other stakeholders on CBOs available within the community, including their services, operating hours, and key contacts.
- Contribute to achieving team objectives by completing additional tasks as required.
