The Hospice Manager will oversee a team of nurses providing compassionate, patient-centered care to individuals in the community, skilled nursing facilities, inpatient units, and those transitioning from the hospital to home. This leadership role involves ensuring the delivery of high-quality hospice care by coordinating patient assessments, developing care plans, and ensuring compliance with regulatory standards. The Hospice Manager will also mentor and support nursing staff, promote interdisciplinary collaboration, and work closely with families to ensure comfort and dignity for patients at the end of life. Effective communication, strong leadership skills, and a deep understanding of hospice care are essential for success in this role.

Responsibilities

Qualified individuals must be capable of performing the following essential duties and responsibilities, with or without reasonable accommodations:

  • Manage the day-to-day operations of hospice teams, including staffing, scheduling, training, and professional development for both hospice and facility staff.
  • Assumes 24-hour accountability for the oversight and coordination of all care management activities.
  • Provide leadership, supervision, and mentorship to department staff across ensuring the delivery of exceptional patient care in alignment with industry best practices.
  • Mentors staff on care planning, case management, visit scheduling, and admission prioritization.
  • Ensure full compliance with all relevant regulatory and accreditation requirements for hospice care, maintaining a focus on quality, safety, and patient satisfaction.
  • Collaborate with interdisciplinary teams, including physicians, nurses, social workers, and chaplains, to develop, implement, and evaluate comprehensive care plans tailored to the individual needs of each patient. Active in interdisciplinary group discussions.
  • Offer guidance, support, and educational in-services to staff in the provision of care to terminally ill patients, emphasizing symptom management, comfort, and enhancement of quality of life.
  • Participate in recruitment, onboarding, and orientation of new staff, and conduct annual performance appraisals. Provide leadership in managing staff performance through coaching, counseling, and the implementation of progressive discipline as necessary, in partnership with the Director of Hospice.
  • Foster a collaborative and communicative environment across multidisciplinary teams, ensuring that all external and internal stakeholders’ needs are met with professionalism and responsiveness.
  • Oversee the fiscal management of hospice services, ensuring adherence to program budgets, productivity standards, visit authorizations, and Charity Care guidelines, while complying with regulatory requirements.
  • Contribute to the strategic growth of the Hospice program by identifying new opportunities and initiatives to ensure its continued viability and expansion.
  • Cultivate and maintain positive working relationships with facility providers, staff, and administration to promote seamless coordination of care.
  • Ensure that patient care documentation, including care plans, nursing assessments, and medication profiles, is timely, accurate, and compliant with agency policies and regulatory standards.
  • Actively participate in the development and implementation of policies and procedures designed to improve and streamline hospice services and patient care.
  • Provide on-call support when necessary to address urgent issues or concerns and ensure continuity of care.
  • Monitor and assess patient outcomes, satisfaction, and overall care quality, working proactively to identify opportunities for improvement.
  • Conducts co-visits and annual supervisory visits to promote quality care and ongoing staff development.
  • Uphold and model ICARE values in all interactions with internal and external stakeholders, ensuring a culture of respect, integrity, and compassion.
  • Perform additional duties as assigned by management.
  • Accountable for meeting the Key Performance Indicators (KPIs) established by the direct supervisor.

Qualifications

Education & Experience - Applicants must possess the following qualifications or an equivalent combination of education and experience:

  • Education: NYS licensed and currently Registered Professional Nurse. Baccalaureate degree preferred.
  • Experience: Required 1 year hospice experience; 1-2 years home care/skilled nursing experience desirable.
  • Licensure, Certification, and/or Credential Requirements: Certification Hospice Palliative Care (CHPN) preferred.

Knowledge, Skill, & Ability Requirements

  • Knowledge of hospice regulations, including Medicare and state-specific requirements.
  • Strong leadership and communication skills with the ability to motivate and guide nursing staff.
  • Ability to manage sensitive and complex care situations with compassion, integrity, and professionalism.
  • Ability to work with patients, families, and staff from diverse backgrounds.
  • Outstanding interpersonal and communication skills both verbal and written.

Technical & Systems/Equipment Proficiencies

  • Demonstrated ability to work with computers and EMRs.
