What are the responsibilities and job description for the RN - Assessment Nurse position at UR Medicine Home Care?
Completes the initial assessment of patients, ensuring appropriate initiation of care plans, services/levels, visit authorization and utilization, and OASIS assessment information that accurately reflects the patient’s clinical, functional, and psychosocial needs.
Responsibilities
Qualified individuals must be capable of performing the following essential duties and responsibilities, with or without reasonable accommodations:
- Develops care plans appropriate to the patient’s health care needs in conjunction with the UR Medicine Home Care (URMHC) Quality team, patient, family, physician, and third-party payers including obtaining medical orders.
- Utilizes resources, supplies and time efficiently in order to contain costs and increase the value of all services.
- Completes the OASIS Assessment to accurately reflect the clinical, functional and psychosocial needs of the patient and other skilled visits as assigned.
- Assures the completion of all appropriate clinical records needed for compliance with state and federal legislation and the delivery of seamless service within established time frames.
- Refers to appropriate Health Care Systems, URMHC, and community agencies/resources.
- Communicates to the health care team members the outcome of the assessment/reassessment (if applicable) visits to ensure appropriate follow up occurs based on the needs of the patient/family’s plan of care.
- Maintains current knowledge of health care reimbursement system and financial impact.
- Maintains positive relationships with physicians.
- Maintains current knowledge of all available URMHC and community-based services.
- Ensures problem resolution at the point of contact.
- Promotes effective working relationships and works effectively as part of the team to meet the goals and objectives of the agency.
- Maintains appropriate productivity requirements. Will complete other visits as needed to meet this requirement including but not limited to ROC’s and re certs and RV.
- Adheres to Professional Home Visit Performance Standards.
- Participates in ongoing refinement of the Start of Care Nurse role/responsibilities to maximize workflow efficiencies and support excellent patient care delivery.
- Meets Key Performance Indicator (KPI) metrics as defined by Clinical Team Manager.
- Other duties as assigned.

Qualifications
Education & Experience - Applicants must possess the following qualifications or an equivalent combination of education and experience:
- Education:
- Experience: Minimum of two-year’ experience in community health experience preferred.
- One year of OASIS experience required
Licensure, Certification, and/or Credential Requirements
- New York State Licensed Registered Nurse.
Knowledge, Skill, & Ability Requirements
- Working knowledge of health care reimbursement systems, government regulations and health industry accreditation standards relative to health care eligibility criteria, payer utilization, prior authorizations, and assessment/teaching standards.
- Ability to effectively communicate with all internal and external health care team members and insurers.
- Excellent organizational/interpersonal skills (including time management, patient visit prioritization).
- Ability to effectively assess the physical and psychosocial needs of the patient/family and develop an appropriate plan of care.
Technical & Systems/Equipment Proficiencies
- Willing to utilize the technology as appropriate to meet the requirements of the job functions.
Salary Range - $73,652 to $86,548
