What are the responsibilities and job description for the RN, Home Visitor - NFP position at UR Medicine Home Care?
Position Summary: Nurse-Family Partnership (NFP) is a research-based program that has been demonstrated to significantly improve critical outcomes for at-risk first-time mothers and their children who receive services in local communities as specified in the NFP guidelines. The three major goals of the program are to (a) improve prenatal outcomes, (b) improve child health and development, and (c) improve self-sufficiency of the family.
Responsibilities
Qualified individuals must be capable of performing the following essential duties and responsibilities, with or without reasonable accommodations:
- Provides home visits to a caseload of 21-25 families adhering to nursing process and the NFP model of home visitation.
- Establishes a therapeutic relationship with clients and their families that promotes self-efficacy.
- Assesses physical, emotional, social, and environmental needs of clients and their families as they relate to the well-being of the family.
- Works collaboratively with client and their families to establish and attain measurable goals in a client centered and inclusive manner.
- Establishes individualized plan of care utilizing clinical nursing judgement and standardized NFP assessments.
- Refer families to community resources as needed.
- Consults and collaborates with other professionals and community agencies actively engaged with clients and families receiving NFP services.
- Documents all nursing care and interactions in accordance with Monroe County Nurse-Family Partnership Protocol.
- Participates in ongoing Continuous Quality Improvement efforts.
- Assumes responsibility for ongoing learning and professional development within NFP practice.
- Performs related duties as assigned or required.

Qualifications
Education & Experience - Applicants must possess the following qualifications or an equivalent combination of education and experience:
- Education: Baccalaureate degree in Nursing required.
- Experience: Two years recent experience in maternal/child health, public health/home visiting or mental health/behavioral health nursing preferred.
Licensure, Certification, and/or Credential Requirements
- Current New York State nursing license in good standing.
- Valid New York State driver's license and automobile insurance.
Knowledge, Skill, & Ability Requirements
- Excellent written and verbal communication skills.
- Ability to speak Spanish is highly desired.
- Ability to establish interpersonal relationships with individuals and families from diverse socioeconomic and cultural backgrounds.
- High degree of autonomy. Ability to impose structure throughout the workday; both in between and during visits.
- Able to complete duties under stress, deadlines and while attending to multiple tasks simultaneously.
- Work assignments require daily travel throughout Monroe County and ability to report daily to office in Rochester, NY.
Technical & Systems/Equipment Proficiencies
- Basic computer skills, electronic health record experience preferred.
Salary Range: $72,618 - $85,072
