Job Title: Maternal Health Support Coordinator/Parent Educator
Location: Petersburg and surrounding counties
Position Overview
We are expanding! Become a part of UBB's newest Maternal Health Hub in Petersburg, VA! The Maternal Health Support Coordinator/Parent Educator will serve as a vital member of Urban Baby Beginnings (UBB), supporting at-risk pregnant and parenting families to promote healthy pregnancies, breastfeeding, parent-infant attachment, and improved birth outcomes. This individual will oversee program logistics, engage clients directly, and ensure access to essential resources. The role involves working collaboratively with Care Coordinator Supervisors and other staff to deliver culturally sensitive, high-quality services while coordinating program activities to meet organizational goals.
This position is not remote and requires in-person work with families in their homes and communities in Petersburg, Va and surrounding counties.
Key Responsibilities
Maternal Health Outcomes
- Maintain a caseload of home-visiting clients, ensuring 90% retention of assigned families.
- Provide culturally tailored social support and educational services, including prenatal, postpartum, and newborn care classes.
- Conduct in-person and virtual visits, offering evidence-based education on prenatal and parenting topics.
- Assist families with birth planning, lactation support, and accessing necessary community resources.
- Organize and facilitate group classes, support sessions, and community events.
- Collaborate with Case Managers to monitor progress and outcomes, ensuring services align with program objectives.
Program Coordination
- Develop and manage program schedules, ensuring timely delivery of services across designated areas.
- Build partnerships with local organizations to enhance outreach and service delivery.
- Track program outcomes and provide detailed reports to leadership.
- Conduct outreach activities, including neighborhood canvassing and community event participation.
- Oversee compliance with organizational policies and state guidelines.
Client Engagement
- Establish trusting relationships with families, offering comprehensive education and support tailored to their needs.
- Connect clients to UBB’s electronic health worker platform and follow up to ensure linkage to resources.
- Foster health literacy and trust in healthcare systems within the community.
- Serve as a consistent point of contact for clients, maintaining regular communication to assess needs and progress.
Data & Compliance Management
- Maintain organized and accurate client files, ensuring documentation is completed within 24 hours of visits.
- Administer screening tools and track program metrics to ensure alignment with performance goals.
- Communicate program trends and compliance issues to the team.
General Staff Responsibilities:
- Participate in staff meetings, conferences, training sessions and workshops as assigned.
- Demonstrate familiarity with employment policies, performance standards, work plan and objectives of program.
- Maintain congenial and respectful relations with staff, children, families and community.
- Maintain confidentiality regarding staff, program, and participant information.
- Maintain objectives and professional standards.
- Improve self-skills and education.
- Perform any other work-related duties as requested by Supervisor or Manager.
- Be present at work in order to provide consistency of services.

REQUIREMENTS FOR ELIGIBILITY
- 2 years of direct patient experience; maternal health experience a plus
- Graduation from Bachelor's Degree program plus 2 years of work experience required; health or human services preferred.
- Schedule flexibility to include day, evening , weekend and on-call work
- Completion of training through Urban Baby Beginnings
- Experience in developing trusting relationships with high-risk prenatal clients and team members preferred.
- Knowledge of women’s health during the childbearing years
- Self-motivated, self-starter, with strong oral and written communication skills.
- Previous home visiting or early childhood education/early head start experience with prenatal, birth and postpartum experience preferred
- Adequate means of transportation with ability and willingness to travel up to 30 miles from office radius.
- Desire to work with children and their families regardless of socioeconomic status or housing situation
- Current physical examination, immunizations including TDAP, and TB screen documentation upon hire & signed by medical provider
- Active and unencumbered driver’s license automobile insurance
- Ability to travel within 45 mile radius of maternal hub
- Background and sex offender check clearance
- Ability to establish relationship of trust and respect with staff, families, and children.
- Strong facilitation and presentation skills
- Strong communication and writing skills
- Experience collaborating with a variety of staff
- Strong attention to detail
- Experience in working with communities of color and strong racial, disability, gender and economic justice framework
- Commitment to anti-oppression and de-colonization work with a focus on racial equity and reproductive justice, specific to birth justice

- Aptitude for solving problems with creativity and resourcefulness
- Ability to develop positive relationships with clients, home visitors, providers, staff
- Proficiency with Microsoft Office Applications, especially Excel and Word, Google
