Job Overview
We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will be responsible for providing support and assistance to individuals and families facing various challenges, including mental health issues, disabilities, and life transitions. This role requires a strong commitment to patient care and the ability to work collaboratively with other healthcare professionals to ensure holistic support for clients.
Duties
- Conduct assessments to identify client needs and develop appropriate intervention plans.
- Provide crisis intervention services to individuals in distress, ensuring immediate support and resources are available.
- Manage cases by coordinating services and resources for clients, including referrals to community resources.
- Implement therapeutic techniques such as Cognitive Behavioral Therapy (CBT) to support clients in achieving their goals.
- Maintain accurate medical documentation in compliance with HIPAA regulations.
- Collaborate with multidisciplinary teams to enhance patient care and outcomes.
- Engage in research data collection to contribute to program development and evaluation efforts.
- Work with individuals with disabilities, providing tailored support that promotes independence and well-being.
- Advocate for clients’ rights and access to necessary services, ensuring they receive comprehensive care.

Hours: Flexible- Telehealth and on-site in Covina. Typically 10am-6:30pm Mon-Fri
Familiarity with E-Clinical Works EHR a plus
Job Type: Full-time
Pay: $75,000.00 - $80,000.00 per year
