What are the responsibilities and job description for the Facilitated Enroller (Bilingual Spanish/English) position at Urban Health Plan, Inc?
Position Summary:
We are currently hiring a Bilingual (Spanish/English) Facilitated Enroller to join our Outreach & Enrollment team at Urban Health Plan. The Facilitated Enroller provides community outreach and assists individuals in successfully applying for and enrolling in Health Insurance programs that fit their needs.
Minimum Qualifications:
- High School Diploma required.
- Minimum of 2 years of experience with clerical/administrative functions is ideal but not required.
- Bilingual Spanish/English is required.
- Must have excellent communication skills and ability to provide exceptional experience to patients.
- Understanding of phone etiquette and standard office procedures.
- Ability to thrive in a fast-paced environment.
Key Duties:
- Secure and maintain training credentials as a Facilitated Enroller.
- Coordinate and collaborate with the Facilitated Enrollment local and statewide teams as well as with other Urban Health Plan program staff.
- Facilitate ongoing monthly community outreach that aims to educate agencies and community members about the Facilitated Enrollment Program.
- Facilitate phone outreach to individuals who qualify for Medicare and who may wish to receive assistance with the Medicare program.
- Professionally handle incoming inquiries and referrals by providing information about the Facilitated Enrollment Program and other relevant services.
- Coordinate with and assist individuals to facilitate their complete application and enrollment process. This process can be accomplished in-person, by phone or via virtual meetings.
- Track and maintain records of completed outreaches and applications to ensure achievement of monthly quotas and reporting deadlines.
- Ensure timely and accurate reporting of program data for both specific program requirements as well as State and Federal reporting requirements. Submission of complete and accurate applications.
Starting Compensation: Minimum $20.67 per hour; Maximum $20.67 per hour

Work Environment Type, Location, and Schedule:
Applicants must be comfortable working onsite, based out of the Bronx and have availability to commit to a schedule of 40 hours per week, working Monday - Friday 8:00am - 5:00pm with some Saturdays as needed.
*Should be comfortable providing coverage at UHP sites located in the Bronx, Central Harlem and Corona,Queens.
