What are the responsibilities and job description for the Screener/Navigator (Bilingual Spanish/English) position at Urban Health Plan?
Position Summary :
We are currently in search of a Screener / Navigator who will be responsible for conducting Health- Related Social Needs (HRSN) screenings, eligibility assessments, and navigation activities for Medicaid Fee-for-Service and non-eligible Managed Medicaid members.
Minimum Qualifications :
- High School Diploma required.
- Minimum of 1-2 years of experience with experience in healthcare, case management or patient advocacy.
- Bilingual Spanish / English is required.
- Must have excellent communication skills and ability to provide exceptional experience to patients.
- Understanding of phone etiquette and standard office procedures.
- Ability to thrive in a fast-paced environment.
Key Duties :
Starting Compensation : Minimum $40,000 per year; Maximum $40,000 per year
As a full time associate at Urban Health Plan, you will enjoy :
Work Environment Type, Location, and Schedule :
Applicants must be comfortable working in an intermittent work environment primarily from home and onsite when needed, based out of the Bronx and have availability to commit to a schedule of 40 hours per week, working Monday - Friday between the hours of 8 : 00am - 6 : 00pm.
