What are the responsibilities and job description for the Social Care Network Supervisor (Bilingual Spanish/English) position at Urban Health Plan?
Position Summary :
We are currently in search of a Social Care Network Supervisor who will be responsible for implementation of SCN services, as well as training staff and ensuring that services are being delivered effectively. The SCN Supervisor will also conduct quality assurance to ensure all policies and procedure are being followed before billing is submitted.
Minimum Qualifications :
- Bachelors degree required.
- Minimum of 1-2 years of experience with experience in healthcare, case management or patient advocacy.
- At least 1-2 years of supervisory / managerial experience.
- Experience with training and development, performance monitoring.
- Bilingual Spanish / English is required.
- Must have excellent communication skills and ability to provide exceptional experience to patients.
- Understanding of phone etiquette and standard office procedures.
- Ability to thrive in a fast-paced environment.
Key Duties :
Starting Compensation : Minimum $68,000 per year; Maximum $70,000 per year

Work Environment Type, Location, and Schedule :
Applicants must be comfortable working in an intermittent work environment primarily from home and onsite when needed, based out of the Bronx and have availability to commit to a schedule of 40 hours per week, working Monday - Friday between the hours of 8 : 00am - 6 : 00pm.
