We are seeking a talented and dynamic Clinical Care Coordinator to join our team!
The Clinical Care Coordinator serves as a vital bridge between the facility, marketing, and its clientele, as well as referral sources. With a dual focus on both nurturing client relationships and ensuring a smooth, continuous flow of referral communication and follow-up .
SHIFT
Mon-Fri 8 : 00a-4 : 00p or 9 : 00-5 : 00p
DUTIES / RESPONSIBILITIES :
- Develop and maintain relationships with the existing referral sources , such as hospitals, medical professionals, therapists, and community organizations for the purpose of streamlining patient care.
- Responsible for ensuring that coordination of care between clients and referral sources are maintained as deemed appropriate by client.
- Act as the point of contact for referrers as it pertains to patient care.
- Coordinate and conduct regular check-ins with referral sources , for patient updates, addressing any concerns and gathering feedback.
- Maintain strong rapport with counselors and clientele.
- Follow up with client's post-treatment to gather feedback, testimonials, and success stories and to ensure successful transition into their next level of care.
- Coordinate with the facility's care and administrative teams to implement suggested improvements when applicable.
- Collaborate with the marketing team to share, develop, and implement strategies and improved systems monthly.
- Maintain accurate and updated records of all client and referral source interactions in the facility's EMR and CRM system.
- Attending discharge meetings to coordinate with referral sources.
- Assist with discharge planning and client discharge plan completion as needed.
